 Calculating Risk (P 4.1, 4.2)

Risk Assessment is the process of evaluating a hazard to determine the level of action required to reduce a risk to an acceptable level. There are several methods have been developed for risk assessment. “Risk assessment methods are used to decide on priorities and to set objectives for eliminating hazards and reducing risks.” Here the follow methods are most popular methods to assessing risk.

a)    Likelihood-Consequence matrix

b)    Risk Score Culculator-1

c)    Risk Score Calculator-2

Given assignment 03 tasks required us to do the risk assessing by method of Risk Score Calculator-2. So we necessary to know what are the principle of Risk Score Calculator-2. This calculator is from Quantitative RA method (i.e. There are three types of methods have been developed for RA Qualitative methods, Quantitative methods and Semi-Quantitative methods). Here the risk of a given hazard can be an equation of;

                        Risk=  Exposure × Probability × Consequences.

Above the equation should be solving step by step. First we want score the Exposure by given equation. The equation says us,

Exposure = Number of employees exposed × Rating

Then the probability is scored by following equation.

Probability = Number of employees exposed × Rating

Finally Consequence scored by the following equation.

Consequence = Number of employees exposed × Rating

Risk score calculator

Ø  Step 1

Scoring the Exposure

In exposure table…..

Rarely rating = 2

Number of employees exposed = 60

Exposure=Number of employees exposed x rating

so

Exposure=Number of employees exposed x rating

                     =2x60=120

So exposure = 120

Ø  Step 2

Scoring the Probability

In probability table….

unusual sequence or coincidence rating =5

Number of employees exposed = 60

Probability = Number of employees exposed x rating

So

Probability = Number of employees exposed x rating

                       = 5x60=300

So probability = 300

Ø  Step 3

Scoring the Consequence(s)

In consequence table…..

Several Fatalities rating = 600

Number of employees exposed = 60

Consequence(s) = Number of employees exposed x rating

Consequence(s)= Number of employees exposed x rating

                                = 600x60 = 36000

So consequence(s) = 36000

Risk Score for Falling from Heights

R = (ExPxC)

R = 120x300x36000

R=1296000000

Task 02

Risk Score for falling from heights is 1296000000.it is so high. It is above K1 (K1=100,000,000).

1296000000 > 100,000,000…

Because the Risk is high.

Possible outcome(s)

Severity/fatality, cause of hazard, likelihood of hazard & the safety setup are the key factors determine the outcome & its impact of that accident. Thereby those criteria are assessed in detail.

 Severity/fatality assessment.

In Severity assessment..two key factors that increase or decrease the fatality of the accident are assessed.

  • How fatal/ severe is the accident?
  • What is the cost of accident? Labor cost, Material cost, Time cost?

Consequences of the accident are the outcome of a risk. So Consequences is an incident can be the measure of the expected severity of that incident. Thereby the Consequences of  the possible accident are assessed. When assessing the Consequences of an accident ,the most severe category one could reasonably expect to result from that accident should be selected .the Consequences of an event can be categorized as follows.,

Fatal / Catastrophic

Death or permanently disabling injury or illness that would prevent return to normal work. this category may also include serious injuries if faced by many people.

Serious

Severe deliberating injury or illness. Such an injury might prevent return to work temporary. Injuries or health issues require hospitalization & long medical treatments comes under this category.

Moderate

Significant  injury or illness requiring more than first aid. Although lost time may result, the injured party would be able to return to work at the same job within a short period of time.

Minor

No injury/property or equipment damage or slight injury requiring no more than first aid. This would mean little or no time delay.

Risk Control Measures (P 4.3)

Before analyzing Risk Control Measures we necessary to know “What is risk control?”

Risk Control

 The process of implementing measures to reduce as far as reasonably practicable the risk associated with a hazard. The control process must follow the control hierarchy, in order, as prescribed in some health and safety legislation. It is important that control measures do not introduce new hazards, and that the ongoing effectiveness of the controls is monitored.

Risk

Vulnerability

Hazard

Any substantial measure that can practically execute and continue that will avoid or at least minimize the severity of a possible threat to safety and health is recognized as a risk control measure.
